If your document has a space for Transaction Date, or just Date, Use today’s date
If your document requires a signature, it should be signed by either an officer or an authorized representative of your tax exempt organization as indicated on the form.
If my exemption certificate allows me to choose Blanket or Single Purchase, which choice best fits my telecommunication services?
If you routinely make similar purchases or purchase reoccurring services, you should check blanket certificate. This allows you to give your seller one exemption certificate to cover all similar purchases, rather than a separate certificate for each purchase. If you choose Single Purchase, only one month’s phone bill will be tax exempt.
The duration varies by state and by the type of tax exempt organization you are. Most state tax exempt documents will show an expiration date, if it will expire; otherwise there is normally no expiration date. However Verizon will periodically request documents to be updated if they are more than three to five years old. Federal excise tax exemption forms for quasi-government and a service person in combat must be renewed annually. Quasi-government would be Volunteer Fire Departments, Rescue Squads, Ambulance, etc. as noted on Form 2506-11, Category 8.
If the documents submitted are completely and correctly filled out, exemptions should be applied within 30 business days or one full billing cycle after the Tax Exemption Group receives them.
The following are possible reasons that you are still seeing tax on your bill:
- You may need to wait a full billing cycle to see the change on the account.
- The documents submitted may have not been filled out correctly/completely and therefore the tax exemption could not be applied.
- The account must be billed in the tax exempt organization’s name which must match your Verizon billing account name or the tax exemption cannot be applied. Please call 1-800-Verizon (1-800-837-4966) to update your billing account name.
- Since various services (i.e., local, long distance, FiOS/Broadband, etc.) are taxed independently of each other, all of the required documents may not have been provided and only a partial tax exemption is in place.
The terms Seller, Vendor, Lessor all refer to a Verizon Service Provider. However the name Verizon just by itself is not the full Seller, Vendor or Lessor name. For instance the full name could be Verizon New Jersey Inc., Verizon New York Inc. and/or Verizon Long Distance LLC. Certificates addressed to ‘Verizon’ will not be accepted. Certificates with multiple Verizon entity names on one certificate will not be accepted. Please see the list at the end of the Frequently Asked Questions for all of the Verizon legal entity names.
Verizon provides a variety of services which may require separate documents made out to different Verizon legal entity names (please see the list at the end of the Frequently Asked Questions for all of the Verizon legal entity names) but the tax exemptions may all be requested at the same time. For instance, if you subscribe to FiOS, Voice and Long Distance, you may need to send in three separate documents.
The tax exempt organization name must be provided on the form. It may say Name of Purchaser, Billing Name or Exempt Taxpayer’s Name and must match the Bill Name on the account.

If your particular state tax exemption document has a field for a description, you will need to write in a description of the services provided on that account. For regular phone service you can write in: Telecommunications Services. If we are providing you with equipment, you can write in Telecommunications Equipment, etc.
Most non-profit companies as well as federal, state and local governments are exempt from state sales tax. Non-profit agencies that are exempt from state tax are normally provided with tax exempt documents or credentials by their state. Typically only non-profit hospitals and non-profit educational organizations as well as governments are exempt from federal excise tax. The federal excise tax is not the same as federal income tax. You can download the federal excise tax document from this website for a complete list of eligible customers.
If you have advised us that you are a tax exempt customer, have provided us with completely filled out valid tax exemption documents which match the bill name on your account and we have not applied the exemption, you can call the Verizon Business Office at 1 800-VERIZON (1-800-837-4966) to request paperwork be submitted for a credit to your account.
