Subject to the Conditions below, a Customer who has subscribed to a new Verizon master service agreement (“Agreement”) may terminate such Agreement (and all services under it) at any time within 90 days of the Services Effective Date by providing written notice of termination to Verizon within that 90-day period.

Conditions of Eligibility:

Other Conditions:

1.   Customer must provide Verizon with at least 30 days written notice in advance of its requested date of termination in accordance with the Notice provision in the Agreement.  (Longer notice may be required for international service.)  Verizon will implement that termination within 60 days of the date it receives Customer’s notification.

2.   Customers who terminate service(s) by invoking this 90 Day Satisfaction Guarantee will be required to (i) pay for any services received under the terminated Agreement through the termination date of such services; (ii) repay all credits, including installation credits, received up to the termination date of affected service(s); and (iii) repay the pro-rata value of any promotional benefits for which Customer has not met the full requirements for those benefits (including without limitation any credits already received or charges waived).  However, Customer’s obligation, if any, with regard to an Annual Volume Commitment (“AVC”) applicable under the Agreement shall be waived to the extent such AVC is affected by Customer’s termination of service hereunder.

3.   Additional conditions may be set forth in the Guide.
