Unfortunately, it’s been more than 60 days since your Fios® service was disconnected; therefore, your account is no longer eligible to receive an early termination fee credit. Due to the time that has passed since your service was disconnected, we’re unable to accept any documents at this time.

We are sorry to hear that we are not able to provide you Fios service at your new location.

To receive a bill credit to offset the cost of your early termination fees for disconnecting services prior to the expiration of your contract, please complete the form below and upload a copy of a document that confirms your new address. Please ensure your full name (must match the name on your Verizon account) and address are clearly decipherable, dated within six months of the disconnect date, and submitted for review within 60 days from the disconnection of your service. Examples of acceptable documents can include but are not limited to a utility bill, a government issued id, or a voter registration card.

After your Fios service is disconnected, please provide the following:

  • Please enter a valid 10 digit number
  • Account number is invalid. Please re-enter (E.g.: 1234567890001)
  • Please enter a valid date in MM/DD/YYYY format
  • Please enter your email address
  • Please upload a copy of a document that confirms your new address.

Once we receive your information, we will confirm eligibility or follow up with any questions within 7 business days.

After we have verified the new servicing address is outside of our Fios area, a credit for the full early termination fee will be applied to your account and will be reflected in your final billing statement. A confirmation email will be sent when the bill credit has been applied.
