Notice of Dispute form

This Notice of Dispute form satisfies the requirement under the Verizon Customer Agreement to provide Verizon with notice of your dispute at least 60 days before filing for arbitration. If we are unable to resolve our dispute within 60 days after submitting this form, either party may proceed to file a claim for arbitration with the AAA.
