This Notice of Dispute form is used to notify Verizon of your intent to file a small claims case or initiate arbitration to resolve a matter between you and Verizon. The form satisfies the requirement under the Verizon Customer Agreement to provide Verizon with notice of your dispute at least 60 days before filing for arbitration, which requires a fee. If we are unable to resolve our dispute within 60 days after submitting this form, either party may proceed to file a claim for arbitration with the AAA.
