How to Apply for Lifeline/Link Up:

1. Choose the state below where you reside, print and complete the application.

2. Mail or Fax your application along with all the required documents.
   
   Be sure to complete all of the information and sign the application where indicated. Applications that aren't completely filled out, legible or signed will be returned.
